Clark County Health is great when working with them on referrals and are very quick and efficient! They have been a pleasure to partner with for Country Terrace in Stanley!

Update: After I reported the facility to DQA they now communicate with families and stopped eavesdropping.

The staff are very loving and caring to their residents. They treat them like family. The residents are always kept busy with tons of activities, events, and outings. The food smells good anytime I've visited as well. The facility is meticulously clean. They care so much, its to a fault. They act like the families are out to get them!

I told my father I wanted him to have a second home. The eavesdropping staff called his guardian and told them I wanted to move him out the facility. This is NOT true! I am buying a cabin with my husband in the area so we can have VISITS. Now the guardian won't let me gain guardianship because of the employees actions. This isn't the first time this has happened.

The social workers avoid phone calls and never call family back. My father has been here for almost 10 years and the family has yet to meet the social worker. Family is not included in ANYTHING. I have to beg for information from the abusive guardian which we rarely get either.

They take wonderful care of their residents. They treat families like poop and make up lies about their intentions, social worker never returns phone calls.

The rehab and nursing staff went above and beyond to make sure my mom was comfortable and felt safe. I feel the facility was always clean and welcoming. I would not hesitate to recommend this facility to anyone needing rehab or nursing home care.

My brother is at this facility on 3 east.
I call and get the answering machine and leave a message but they never give tell my brother that he is supposed to call me. He has asked to used the phone to call me for three hours and they will not him the phone. It ends up that we don’t talk that night.
Also my brother is hard of hearing so he has to have the speaker on the portable phone and the staff listens to everything he says.
I know this because one time I told him to call a realtor and the person that heard there when and called his GUARDIAN, because I received a call from her. I don’t appreciate the staff eavesdropping on my brother conversations.
My brother told the staff for at least two weeks that his tooth was hurting him, and they did NOTHING until I called a group called OMBUDSMANS for NURSING HOMES. They work with patients that are having problems at a NURSING HOME. Since then my brother has seen a dentist. But his guardian said that they were not sure when they could take him because a CNA had to go with him and they were short on staff. My friend who is an RN OFFERED TO TAKE HIM and then they found someone really fast to take him to the dentist. My brother tooth is now black.
I could go on and on about this facility and how bad it is. I hope I can get my brother out of there and closer to where I live which is the La Crosse area.
Please DO NOT SEND YOUR LOVE ONE THERE , ESPECIALLY IF THEY HAVE TO BE ON 3EAST.
One nurse told me that my brother was CRAZY AND IMPLIED THAT HE WAS CRAZY 24/7.
,

About this data
How to read this page
Star ratings vs. Care Quality Score: The Google star rating reflects visitor and family opinions collected from public reviews. The Care Quality Score is an independent metric derived entirely from federal inspection data — the two measure different things, and comparing them side-by-side gives a fuller picture.
Where this data comes from
All clinical data on this page originates from the Centers for Medicare & Medicaid Services (CMS), the federal agency that certifies and inspects every Medicare- and Medicaid-participating nursing home in the United States. Reviews are imported verbatim from public Google business listings.
What to do next
Data is a starting point, not a final answer. We encourage families to visit facilities in person, observe daily routines, and speak with staff and residents. See our guide: Questions to Ask When Visiting →
