This is a horrible place. Residents are neglected, ignored, and left suffering. One of the last times I visited my grandmother here, she was crying because she was so thirsty for water. I would spend hours there without one employee coming in to check on her or her roommate. They have no empathy or compassion. It is a very dark and depressing place.

If I could give zero stars I would.. working in the health care field myself I was extremely disappointed with the care my grandmother received. A family member should not be worried about their loved one upon leaving wondering when the next time someone is going to check on them, offer water, help with eating, toileting and so on. I would visit for hours and not a single staff member would come and check on the residents for HOURS. Letting them sit alone. I took it upon myself when I would visit to help my grandmothers roommate. She needed help with drinking water and turning her tv on etc.

I came to visit one day and my grandmother was in tears from how thirsty she was. Not to mention she was in her transitional phase end of life. Her oxygen concentrator empty every time I visited. Like I said it was so hard to leave her every time.

If a staff member of this facility is reading this, I hope you offer the appropriate training to the care staff in all stages of care. The resident to staff ratio was below normal. Not to mention the lack of attentiveness to these residents. Whenever I tried to look for someone to assist my grandma or even get some cold fresh water no one was to be found. When I did finally hunt someone down they were rude and acted like I was bothering them. Making it extremely uncomfortable.

I’m honestly at peace my grandmother transitioned to heaven and no longer has to suffer alone when one of us grandchildren are not there.

If you have a loved one living in this facility please check on them often. I’m surprised state has not stepped in at this point.

I was a short term rehab resident and I'm now at home. I can't remember everyone's names ..... Sorry. I just want to say THANK YOU to the CNAs, RNs, PTs, NPs & the rest of the staff for your excellent care.

My Mom was admitted to the Pavilion on June 20th on June 21st she was given a shower! On June 28th we started ?? When her next shower should be, none of the nurses seemed to have an answer! She now as a bed sore from uncleanliness! I walked into the social workers office on June 30th I spoke to Jamie and the head nurse and they were supposed to get me answers! Mom fell again and I received a call from a nurse and I than asked that nurse and she told me she hadn’t had a shower since June 21st! I appreciate that nurses honestly! That nurse also said she would make sure Mom got a shower that evening! I called the home on July 2nd and spoke to NP Katie and expressed my frustrations and said I would be taking her out that day! I walked into the head nurses office and tried to find out what her review showed and why I still didn’t have any answers on why she hadn’t had a shower in 11 days , she was very rude and tried to come up with many excuses and I told her that this is pure laziness and she said our conversation was over! I than set up a face to face meeting with Dawn the Administrator on July 2nd after I moved Mom back to her assisted living! I was told by Dawn the Administrator that she would investigate and get back to me early the week of July 6th! I called and left a message for Dawn the administrator on Friday July11th and Monday July 14th on her voicemail and still no call back! I will be filing a claim with the state of Wisconsin! So sad that my Mom received such horrible care and hope this deter’s anyone from placing a family member there!

About this data
How to read this page
Star ratings vs. Care Quality Score: The Google star rating reflects visitor and family opinions collected from public reviews. The Care Quality Score is an independent metric derived entirely from federal inspection data — the two measure different things, and comparing them side-by-side gives a fuller picture.
Where this data comes from
All clinical data on this page originates from the Centers for Medicare & Medicaid Services (CMS), the federal agency that certifies and inspects every Medicare- and Medicaid-participating nursing home in the United States. Reviews are imported verbatim from public Google business listings.
