My mother was a patient here. The main reason for the negative review are two factors;
Improper record keeping-
1.her discharge papers for her transfer were incorrect and if left uncorrected by ourselves could have lead to a disastrous outcome
2. Lack of communication from the facility. I called to let the social worker know that the discharge documents were incorrect. The social worker never returned my call. I later followed up with a director and left a message and never received a return call.
Other than these two issues, there was both good and bad care. There were terrific staff members who were very helpful and courteous there were other staff members who were inattentive. There were times when staff responded quickly to a request for help and other times when my mother was left unattended to after calling for assistance.

My grandmother got discharged from Wausau to this facility, when I tell you the photos they have up are very deceiving!!! I mean very deceiving. Looks nothing like the pictures. Very outdated and unprofessional. She arrived at 2:30 pm and left against medical advice at 5pm central time. They refused to fed her because she missed lunch because she was being transported. When I arrived at the facility I was so disappointed because it looked NOTHING like the pictures. Honestly, I’m happy I took her home. A bird flew into the window while she was standing up to leave. Definitely a sign.

Care here is very much hit and miss. Some staff do care, while others seem to be more interested in passing time until their shift is over. Response time after pressing a call button is abysmal. 30 minutes or longer is not unusual. This is very disheartening when you are waiting with your loved one. Do not press the call button during a meal, because staff is more interested in the task at hand. If you need help, you may very well have to just track staff down and be assertive, sticking up for your loved one. It’s a sad situation.

When a person who has cancer asks REPEATEDLY for their pain medication and was ignored, that is beyond unacceptable. It should not take a visitor to chase down staff and get on them about doing their job in order for the staff to get the patient their pain meds. The patient had been asking for a lengthy period of time. No pain meds were given until I personally went to find staff members to ask why she wasn't given her pain meds, despite multiple requests. I was in the room when one of the requests was made. The staff member said she was going to get a nurse to get the pain medication. We waited, and yet nobody came. It breaks my heart to think about how long the patient would have continued to wait had I not stepped in to try to help. Also, she was fed through an NG Tube. She was constantly getting wet because of it coming undone. When I walked in to visit her shirt was covered from the liquid in the tube and it was completely DRY. In order for that to happen, that would take a lengthy amount of time. That's how you leave your patient? It takes very little time to change a shirt. As a former CNA, I am beyond furious and sickened by what I witnessed in this facility. Failing to do your job as Medical Staff is unacceptable! Health care providers take an OATH to take good care of the patients. Being short staffed is NEVER an excuse for what in my opinion was medical NEGLECT by the staff!

About this data
How to read this page
Star ratings vs. Care Quality Score: The Google star rating reflects visitor and family opinions collected from public reviews. The Care Quality Score is an independent metric derived entirely from federal inspection data — the two measure different things, and comparing them side-by-side gives a fuller picture.
Where this data comes from
All clinical data on this page originates from the Centers for Medicare & Medicaid Services (CMS), the federal agency that certifies and inspects every Medicare- and Medicaid-participating nursing home in the United States. Reviews are imported verbatim from public Google business listings.
