I relocated my 76-year-old mother to this facility in February 2025, following all the proper procedures and coordinating the move through then-admissions coordinator Ms. Baker. Unfortunately, what was already a delicate transition for my elderly mother turned into a stressful and disorganized experience.<br><br>On the day of her arrival, the staff had no idea she was being admitted. Her room was not ready, her medical records were not available, and the care team had no knowledge of her medications or care needs—despite this being part of the admissions process. My mother, elderly and frail, was forced to wait in an uncomfortable wheelchair for an extended period of time. This lack of preparation and empathy was unacceptable.<br><br>A few months later, while being transported to physical therapy in a wheelchair without footrests, my mother slid from the chair and fractured her femur. I was never notified by the facility—my mother herself had to tell me. When I called for answers, I was told an incident report had been filed and that it was “an accident.” The response felt dismissive, and staff only promised future training—something that should have already been implemented to ensure basic resident safety.<br><br>The most heartbreaking moment came on July 20, when I was informed that my mother had been found unresponsive and transported to the hospital. She was later pronounced deceased. While I understand that emergencies can occur, the way this was handled was deeply troubling. As an out-of-state daughter, I called the facility a few days later with my daughter to request clarity about what happened and how often my mother had been checked on prior to being found unresponsive.<br><br>We spoke with a staff member (Toya or LaToya) who admitted she was new and unaware of any policies or procedures. We were then transferred to the Director of Nursing, whose tone was cold and defensive from the beginning. When we inquired about documented wellness checks, she stated that “notes aren’t always taken” and that “there is no policy or procedure for informing families.” When I asked if families are ever contacted or even offered condolences, she bluntly said, “No,” and then shockingly asked, “Why are we having this conversation?”<br><br>I do not blame the facility for my mother’s death. But families deserve transparency, compassion, and communication after traumatic events. The lack of professionalism, empathy, and proper protocols we encountered only deepened our grief.<br><br>In my opinion, the policies, procedures, and overall standard of care at this facility should be thoroughly reviewed by the State of Alabama. Vulnerable residents deserve better, and families should never be left in the dark or treated without basic respect and decency.

If I could give them negative stars then i would! My mom passed away there 5 months ago; i went to visit her the day she was transfered there from the hospital for rehab and they tried telling me she wasnt there yet and once they decided she was, they had her listed in the wrong room; then they had no clue where she was.. they had to call another employee that wasnt there to see if she knew. That should have been when i took my mom somewhere else, but i gave them the benefit of the doubt; big mistake!! while she was there (less than a week) they had her medicine messed up, it seemed to be an issue when she asked for ice and half of the time she never got it, most of the staff acted like they didn't have a clue what they were doing, and my mom's bipap machine was never once used even though it was supposed to be on her every night. I kept asking and they said someone would take care of it; that never happened. When patients needed help with anything, it took the staff forever to respond, and when i called up there and asked to speak with her nurse, she was busy and never called me back even though i made it clear that i needed to talk to her about my moms blood pressure being extremely high. (That was the day before she passed away). When my mom passed away, we asked questions about it and nobody gave us straight answers and when I made a written request to get my mom's records, I never received them. I can say that the therapist were good, and there was one staff member that we did really like but i do not remember her name. But i do not recommend this place at all!!!

Would not recommend. Some of the staff were great, but it seemed like everyone we talked to had no idea what to do or what was going on. More than four people told me they had just been hired. Did not feel like the staff was able to keep people clean and help them get better. We had an emergency, pushed the help button and no one came. And when I ran out into the hallway for help, the staff was rude. Dining unable to accommodate dietary restrictions or allergies. PT staff were helpful. And the building is nice, it smells better than some of the other ones.

My father is currently here for rehabilitation. The cleanlines and level of care is unmatched. I highly recommend this facility if your family is in need. Every staff member is compassionate and caring. They place their patients first.
