I wish that I could select -1 stars this entire facility is nothing but liars and manipulators. Suppose to be social worker name Gwen all she does is lie and she isn't even certified she's an assistant but she almost always overstep the actual SW. My dad went an entire shift with no medicine and was later asked if the medication was even administered by the DON no one even came in and checked on my dad the entire night call light remained on for over 30 mins and was told someone had to find the nurse on the hall this was around 830-9 pm on January 1st 2026. Later to find out by my surprise the nurse stole multiple patients medications. I later learned from another staff that nothing happened to the nurse and she returned to work the following day without any disciplinary action also that same staff member said "oh nothing will happen they hide everything here even the fact that the activities lady came in drunk twice around staff and residents". I wouldn't send my dog to this facility. It most definitely needs to be investigated by the state because this is beyond unacceptable. The administration here is a joke they are not in the right business because they have no regard to human life. Therapy was great. Hannah was my favorite she's good at what she does. This building should be closed down.

I don't like how my Daddy was treated at Rocket City Rehabilitation and Healthcare Center. This may not be applicable to all rehab facilities or healthcare caregivers, but it is specific to Rocket City Rehab. My Dad entered their facility in August 2025. Ever since then: 1) He still has unexplained cuts and burns. There's still a big scar/burn still healing on his right arm that not one nurse at that facility can explain how it got there. 2)The patient floors remain filthy and sticky and no, this is not due to current renovations. 3)1st instance, he was sent to the ER from the facility and we as the family was not alerted until almost 2 hours later. 4) 2nd instance, he was sent to the ER from the facility without ANY identifiers on him. No arm band, name tag, name badge, sticky note, dog tag, collar, necklace, bracelet....NOTHING. They sent him to the ER as John Doe for no reason. 5)They felt that withholding all of his belongings was a sufficient business practice because "no one knows where anything goes or where patient belongings go." It took 38 minutes of unnecessary banter and disagreements to be allowed to retrieve all of his belongings because "I don't know" is the standard response to all questions there. Ignorance is bliss. 6)They felt the better option was to leave him cold in his room instead of clothing him in the pajamas or warm attire that we as a family provided to him for warmth and comfort. 7)They're oblivious and inconsiderate. Having a loved one in a facility is hard. Learning that the facility is purposefully negligent is frustrating, especially while emotions are already high. Avoid Rocket City. Govern yourself accordingly. Good day.

Well this place is a HOT MESS on the outside of this facility! I am seeing lots of violations inside the facility which they claim they are remodeling with 22 mil non-commercial grade laminate flooring in a Nursing Home which won't last but a year. really they need a new roof you can see the drainage stains remain .apparent that the dry rot is cracking on the exterior of the building...I can smell the black mold inside the walls...

My mom was here for 2 weeks before being sent to Hospice. The nurses, nurse assistants, food service, financial, and other staff were wonderful and took good care of my mother who could not do anything for herself. The only staff that I did not appreciate was the social worker. She was short, dismissive, rude and seemed burned out or just in it for the paycheck. She was not helpful at all. The social worker is the one who is supposed to be helping families during this difficult and scary time but she was not. The building is old and being worked on but it always smelled clean.

This place is a hidden gem! They have taken such good care of my friend Frank since he's been here for the past few months. The nurses are all awesome!

About this data
How to read this page
Star ratings vs. Care Quality Score: The Google star rating reflects visitor and family opinions collected from public reviews. The Care Quality Score is an independent metric derived entirely from federal inspection data — the two measure different things, and comparing them side-by-side gives a fuller picture.
Where this data comes from
All clinical data on this page originates from the Centers for Medicare & Medicaid Services (CMS), the federal agency that certifies and inspects every Medicare- and Medicaid-participating nursing home in the United States. Reviews are imported verbatim from public Google business listings.
