Do not place your loved ones here. My father was at this facility from December 30 to January 6 following a broken hip. He is diabetic, and every single day I had to speak with nurses or management because he was not being given his prescribed medication. At one point, the Nurse Practitioner advised me to file a grievance, stating that the medication orders were very clear and simply were not being followed. On another day, I spoke with Pam, the Case Manager, who assured me my father would be given insulin. Two hours later, he still had not received it. The care aides are wonderful. They genuinely do what they can with the limited support and direction they are given. This is not a reflection on them. On the second day, a nurse attempted to give my father’s medication to the man sharing his room. This happened more than once. Thankfully, that gentleman questioned it, as he does not receive injections. This was extremely alarming. Physiotherapy was another major issue. I was told my father would receive 30 minutes per day (excluding weekends). In reality, he received two days of physiotherapy, each lasting no more than 10 minutes. The entirety of the therapy consisted of standing him up 2–5 times and then laying him back down. One scheduled physiotherapy day was missed entirely, resulting in yet another full day of him lying in bed. Again, the response was simply, “We’re sorry.” During our time here, I spoke with the Director of Care (Cindy), the Assistant Director of Care, and Pam. The response was always the same: apologies, but no meaningful changes were ever made. This facility consistently failed in medication management, patient safety, and rehabilitation care. I would avoid this place at all costs.

How lucky I was to get a bed at Dessert Blossom. My last encounter at a rehab facility was horrible. I was a nervous wreck entering Dessert Blossom hoping this experience would be an improvement over the last place.I was immediately welcomed by a wonderful CNA named Diane. Her bright sunny personality put me at ease, she explained everything about the facility. I actually felt welcomed as a human being not just another patient. My 10 day stay couldn’t have gone better. Everyone I encountered was so pleasant. I felt that everyone had a genuine concern for the patients. Thanks to all from medical staff to activities staff to therapy staff. You are greatly appreciated!

This place is truly amazing. My mom suffered a traumatic brain injury in 2009 and lived in state-assisted independent housing until 2012, when she fell, broke her spine, and re-injured her head. When I first visited her here, it was really hard—it didn’t feel good, it smelled unpleasant, and I left feeling heartbroken that this was where she was living. Coming back now is completely different. The place smells clean, she’s in good spirits, and the environment feels caring and updated. The remodeling has made a huge difference. Second chances really do happen, and this place absolutely deserves one.

Worst place to go. I would give 0 if I could. When I arrived my driver had to put me in my bed and he didn't even work for them. I had a total hip replacement. They leave people would can't get out of bed in their poop/pee all day. Didn't tell me my pain killers were by request. Thought I was getting them for 2 days and didn't understand why pain was so bad. When I asked the cna (nurses only come in room to give meds and right back out door before you can ask them a question) why and they responded ask your nurse. NEVER answer call light. You will die before they respond. 2 people fell out of their beds and it took over 20 minutes for someone to respond and with yelling and call light. You rarely see a physical or occupational therapist. I can go on and on. This place is bordering on elder abuse. Don't believe the good reviews, there either fake or the people haven't stayed there. Really terrible place. No one cares.

About this data
How to read this page
Star ratings vs. Care Quality Score: The Google star rating reflects visitor and family opinions collected from public reviews. The Care Quality Score is an independent metric derived entirely from federal inspection data — the two measure different things, and comparing them side-by-side gives a fuller picture.
Where this data comes from
All clinical data on this page originates from the Centers for Medicare & Medicaid Services (CMS), the federal agency that certifies and inspects every Medicare- and Medicaid-participating nursing home in the United States. Reviews are imported verbatim from public Google business listings.
