I do not recommend Heritage Court Post Acute to anyone who expects safe, consistent, or competent care for their loved one. During my father’s stay, I had to personally review his medication list multiple times because of repeated errors that could have seriously harmed him. Weekly lab orders were not followed correctly, and I constantly had to remind staff or verify whether labs were drawn at all. Nurses also failed to monitor his calorie intake despite his ongoing nutritional concerns. Communication was extremely poor. I repeatedly had to follow up on scheduling updates often after offering to handle the scheduling myself because no one could give me a straight answer. My father’s low blood pressures were ignored, even though he was symptomatic. When we finally took him to an outside office appointment, the physician immediately recommended he go to the ER due to hypotension and tachycardia, issues the facility should have recognized and acted on long before. This level of neglect is unacceptable. My father did not receive the safe or attentive care that any resident deserves. I strongly recommend looking elsewhere for a facility that takes patient care seriously. Do not send your family members here.

This place is a nightmare, not clean. They don't care about the residents. I would avoid at all costs.

Nurses trying to talk patients and families out of sending worsening patients to the hospital. Not enough staffing left my mom sitting in a soiled diaper all day.

About this data
How to read this page
Star ratings vs. Care Quality Score: The Google star rating reflects visitor and family opinions collected from public reviews. The Care Quality Score is an independent metric derived entirely from federal inspection data — the two measure different things, and comparing them side-by-side gives a fuller picture.
Where this data comes from
All clinical data on this page originates from the Centers for Medicare & Medicaid Services (CMS), the federal agency that certifies and inspects every Medicare- and Medicaid-participating nursing home in the United States. Reviews are imported verbatim from public Google business listings.
