Well if there was a tiger, I know I would not have to worry about being eaten.

My first night at Osborn I thought it was the worst place I could have gone to. All I needed was a little adjustment to my new location. Speaking with my CNA, Boudie, he explained what I could expect. The next day my CNA was Catherine and she was fabulous. Anytime I need to be changed she would bring a helper because of my size. When Catherine and Tyrell were together they had me laughing so much that I had no time to feel embarrassed. The nurses were very attentive to my needs and on top of my medication times. My favorite time in the day was for PT. Jonus was my goto guy. He is great. He will push you to do one more if you think you have it in you. I had many OT therapists. They are all great. The only reason Osborn did not get a 5 star review is because of the meals. That is one place they can improve. If I had to recommend a location for rehabilitation, this is the place to go.

Exceptional staff are very courteous, kind, caring and generous. They are helpful, considerate and compassionate to help in every way possible.

My family and I were incredibly hopeful for this place. My sister heard great reviews from transport about this location, but soon found she was misled. To be fair, the therapists and doctors have been great. However, staff and certain CNAs have been a joke to say the least. Leaving patients in their own filth for hours at a time, not answering patient's call buttons for hours at a time, when they're close to literally falling out of the bed, inedible meals, the list goes on and on. We wish we had the financial capability and options to move our family member, but we don't. Hopefully, this review doesn't fall on deaf ears. All I can say is don't send your loved one here unless you want random people wandering into your room, half naked at all hours of day too, because no one is going to respond to your pleas when you shout at them to get out. All of this is just unacceptable.

I would not recommend this center to anyone who is elderly and has had surgery that requires therapy. There is evidently no communication between nurses and therapists as to how a patient needs to be handled. After 3 weeks my family member came home in worse condition than they went in. Sent home with soiled undergarments and a grade 4 bedsore. They are now totally bedridden.

About this data
How to read this page
Star ratings vs. Care Quality Score: The Google star rating reflects visitor and family opinions collected from public reviews. The Care Quality Score is an independent metric derived entirely from federal inspection data — the two measure different things, and comparing them side-by-side gives a fuller picture.
Where this data comes from
All clinical data on this page originates from the Centers for Medicare & Medicaid Services (CMS), the federal agency that certifies and inspects every Medicare- and Medicaid-participating nursing home in the United States. Reviews are imported verbatim from public Google business listings.
