DO NOT PUT YOUR LOVED ONE HERE .<br>In the past month they have ran out of medication/ didn't get ordered. And managed to lose / misplace it . This has not only happened to my family member but several others who are there as well .obviously management is not on top of taking care of these people . I've filed a few complaints with the medical board as well now so hopefully no one else will have to deal with this and suffer .

In January 2025 my mother was admitted to Trellis Chino. I have to say that this place is everything it appears to be, five stars all the way! Trevor and his team keep this place running smoothly. The layout is perfect for the person who is taking physical therapy here and trying to heal mentally and physically. The physical therapy team does an amazing job. The nursing staff is on top of it. My mother still asks to go back to this beautiful place.

My mother broke her back and had surgery. She came to Trellis after her surgery and was there 3 months. I have been to other rehabs. This one looks like a nice hotel instead of.a dreary nursing home. They have private rooms and baths. It has a good atmosphere. The staff is helpful and courteous. The rehab was great. Always remember to be with your loved one every day and the staff pays more attention if you are there and on top of things, just like in the hospital!

My relative was admitted to Trellis in May 2024 and discharged in early Oct. unfortunately, a prior facility failed to provide appropriate nursing skills and she was verybdeconditioned on arrival.<br>Great team work and careful slow work, she was successful in gaining back strength to continue therapy at home.<br>Kudos to Emily from Speech Therapy; Roya, Robert, Raymond, Lindsay and Eunice from the therapy department. She is back to walking using a walker and soon be walking independently! Love licensed staff RN’s and LVN’s watched over planning and progress- Jodi, Lila, Ben, Gio, Arranza, Liz, Tracey, Adrianna, Tracey, Eddie and Jose were excellent. Nurse aides Mayra, Daisha, Merlin, Maiacela, Jonathan, Guillermo, Anna were so caring and encouraging. You each made a difference!<br>The Administrative team listened to concerns, worked to make adjustments and changes to correct problems.<br>The facility is clean, well maintained and the EVS staff engaged. Food service is a work in progress. Diets are bland, dry and need some work!<br>Thank you all so very much! I recommend Trellis to all who need help and can work hard! Case manager Yuri was key to communication and a clone!<br>All our best!
