DONT LEAVE YOUR LOVED ONES IN THEIR CARE! My brother died in their care 5 years ago because of their neglect. He was receiving care there before Covid restrictions began and he was severely neglected before and during. There was a male aide there who made it well known that he did not like my brother. On more than one occasion my brother discussed with me about how he was discriminated against while living in that facility. While Covid restrictions had been instituted my brother would call us constantly about how the weren’t feeding him or hadn’t brought his food yet. How they had him sitting in his own filth for longer than what should have been acceptable. If they had properly took care of him when he was released from the hospital he could have been saved. Please don’t let anyone you are considering to be in a nursing home stay there. They are deplorable.

Their medical practice its out of dated. They were still giving me the patch for high blood pressure while I was staying in rehab. My cardiologist took me off the patch and prescribed higher dosage medication for high blood pressure instead. Some patients stayed in rehab have highly mental issues. You don't know if they will go off on you one day. They made me stay in the wheel chair for all transactions from moving to one room to the next. Not giving enough exercise for my legs. They put 4 patients to a room. Very seldom 2 to a room if you are lucky. Everyone admitted was giving a wheelchair. Not a good idea because less walking will weaken your leg muscles. Some staffs are NOT very professional. They curse and the reason for a 0ne star. It took 3 months for me to get my leg muscles to be stronger. I don't need to rely on my walker or wheel chair any more. Thumbs up to the physical therapist and occupational therapist to made me stronger by working on my muscles daily. Its just insufficient time spent for therapy.

I have to say that Park Terrace is an incredible rehabilitation and nursing facility! The team that they have there are second to none!

The worst and most dangerous place on par with Pennhurst. Filthy. Uncaring, rude staff. Staff doesnt check if patients bathe or brush teeth ever. Play loud music at 3am when patients should be sleeping. Talk loudly at night as if there’s a party. Mentally abuse and talk badly to patients. Do NOT TAKE ANYONE HERE WHO NEEDS TBI CARE.

WORST PLACE EVER! I was transferred here on November 2023 for antibiotics purposes. The room was dirty and they messed up my antibiotics course because it was never given on time. My permanent IV was never cleaned properly. I ended up with high fever for days & tonsils. I had to go to ER from the rehab because the doctor at the rehab barely treated me or gave me proper medication!! THIS PLACE IS HELL!! And it should be shut down. Your loved ones will suffer. DO NOT SEND THEM HERE.

About this data
How to read this page
Star ratings vs. Care Quality Score: The Google star rating reflects visitor and family opinions collected from public reviews. The Care Quality Score is an independent metric derived entirely from federal inspection data — the two measure different things, and comparing them side-by-side gives a fuller picture.
Where this data comes from
All clinical data on this page originates from the Centers for Medicare & Medicaid Services (CMS), the federal agency that certifies and inspects every Medicare- and Medicaid-participating nursing home in the United States. Reviews are imported verbatim from public Google business listings.
