I have been so pleased with my stay at Bolivar Health & Rehabilitation. The staff are very concerned & encouraging. I am so happy to say that I am making progress with PT with the goal of returning home as soon as possible.

There are several that I wish I could call out for their exceptional work here. When I had to go back to the hospital for a week it was fine there, but I actually missed being at BH&R.

I love that the facility smells clean when you walk in. I have met so many lovely residents that share their stories with me. We also have activities in the cafeteria daily. We sing, play games, do puzzles, have performers, & reminisce about the “good old days”!

Everyone works hard and most everyone is very friendly. Thank you BH&R Staff!! They have worked with me on transport to Doctor appointments and worked with me to get a mobile transfusion company to come here & fill my Baclofen pump before it ran out, which no where else could do! ❤️ I was terrified but they came through for ME!!

I highly recommend Bolivar Health & Rehab for you or your relatives.

Christian Care Center has been the absolute best place I have come across. No nursing home is perfect but this one is pretty dang close. I have toured several facilities and had different outcomes at each facility. Each facility has had its faults. Each facility has had good and bad reviews. I understand that you can not make everyone happy 100% of the time. Where ever you choose to place your loved one then it is up to you to advocate for them. Visit often and voice any concerns that you may have. If they are not addressed to your satisfaction then make it known. Overall it is your responsibility to ensure that your loved one is cared for. If you feel that your concerns are falling on deaf ears then move your loved one to a different facility. If you choose to leave them there then what happens to them is just as much your fault as it is the facility. What you should not do is bad mouth a facility that you chose to leave your loved one at if you didn't think they got the proper care they needed. Your loved one is your responsibility. I can not visit on a daily basis but I can promise you that my loved one is very well cared for at Christian Care of Bolivar. My loved one is thriving and I would not choose any other place for her. Overall 5 stars to Christian Care Center of Bolivar.

From the upper management to the staff, no care is given they are there to do the time and get paid. My dad was found on multiple occasions dirty and soiled while the staff was sleeping in other rooms or LOUDLY partying at the nursing station. I am not trained in the nursing field but, I am aware of manners and dignity these people have none. If you complain it only makes it worse on your loved ones they seemed to take out their anger on them. I would not put an animal in this place if given the opportunity again and due to insurance, location and "Christian Care" though it would be better bit it wasn't the case at all. In closing i just hope and pray that every staff member there receives the same treatment they gave my dad when they get to that age.

I never imagined my mama ever going to a nursing home. I always said that I would never allow it to happen. Well fast forward to a year or so ago... She had a stroke and recovered well from that only to slip on a puppy pad and fracture her left tibia plateau. She went to WTH rehab for 14 days which I think everyone should go to first for therapy!! However, they only allow you to stay 14 days so she was still non weight bearing so we had to find somewhere else for her to go. I work on an ambulance so I know what nursing homes are an absolute NO and which ones were an option. I chose one about 45 minutes away from me and drive past one every day on the way to go see her. I will not say anything bad about that particular one because we simply chose the better of the 2 in our opinions plus the closer one did not have a physical therapist at the time. So we decided on Christian care center of Bolivar. It is cleaner than most nursing homes that I have entered and working on an ambulance, I have walked in several in the last 10 years. They have been great to my mama and myself. I will say this, nursing homes are a necessary evil in life. I do not believe anyone wants to put their loved ones in a home but sometimes there isn't any other option. Mama went for rehab and CHOSE to stay. I tried to convince her to come home and she knows that is an option every single day. They have been great to my mama from the administration all the way to the aides. They constantly have activities and keep the residents involved. They offer snacks through out the day and always have other meals if your loved one does not like the meals they get at lunch or dinner time! The kitchen staff accommodate the resident's preference. For example: my mother does not like fish so on fish days they make her chicken nuggets or something else. The staff are usually on point. The one time mama fell, the staff called me before the ambulance arrived and advocated for my mama to go to Jackson for me. You will not find that anywhere else. The time my mama had a seizure, I was there and the staff was willing to follow my directions for my mama. Now I was disappointed when we moved to the 200 hall but that was because we was so used to our nurses but change is good sometimes and mama gets exceptional care on the 200 hall too. The nurse practitioner is more orientated to patient care than any doctor that I have ever been to. Now I will say for any nursing home, you(the family) need to be an active in actually showing up. I am confident that if I wasn't able to visit that my mama would be loved as their own family and cared for but a nursing home isn't a shelter and your loved one shouldn't be dropped off like a dog at the pound!! So be involved and ask questions!

Did I mention the fact that this place has wound care so those wounds get addressed every single day!

I gave 4 stars because there have been hiccups that have been resolved and every place can stand improvement. Even me taking care of my mama at home stands room for improvement. Show me one nursing home that doesn't have hiccups..

I'll end this review with a chance to educate you! KNOW YOUR RIGHTS!! The hospital will ask you for 2 choices, always know you have options. This place helped mama get tenncare so she could stay long term! IF you need rehab and want your loved one to have aggressive rehab then look into WTH rehab in Jackson. Mama went there twice but you can only stay there 14 days. If you still need rehab then you can come here after 14 days. With Medicare you only get 14 days for rehab usually but WTH rehab doesn't count towards those days because they fall under the hospital stay so you can get double the rehab for the same with Medicare. This is what nobody will tell you at the hospital that we learned from going there twice. Ultimately you get to choose where to go. If they say that place did not accept you then CALL THEM and verify that!! Some hospitals will lie to you because they do not want to call around and just want to pursue the easiest choice that they know will say yes.

About this data
How to read this page
Star ratings vs. Care Quality Score: The Google star rating reflects visitor and family opinions collected from public reviews. The Care Quality Score is an independent metric derived entirely from federal inspection data — the two measure different things, and comparing them side-by-side gives a fuller picture.
Where this data comes from
All clinical data on this page originates from the Centers for Medicare & Medicaid Services (CMS), the federal agency that certifies and inspects every Medicare- and Medicaid-participating nursing home in the United States. Reviews are imported verbatim from public Google business listings.
