My mother was sent to the Madison Health and Rehab Center at 110 Belmont Rd Madison on friday Jan 16th 2026
From the Meriter Hospital at 202 S Park St, Madison.
They put her in a room which had no heating and the washroom was shared with 2 rooms. They did not give her the Pregabalin medication stating that they don't have it and they are trying to get it soon. The staff was very very rude.Next day on Jan 17th saturday 2026 morning my mother called me stating that she's been ringing the bell since 9 am and no one is answering to lay her down into the bed. I called their office several times and then at 11 am somebody showed up to assist. In the afternoon she asked for the same med(pregabalin) and they still don't have that. I visited her around 8 pm for dinner and the staff told that the office is closed and they still dont have the medication.Next day on Jan 18th at 8 am my mother called me and said that her sugar is very high and there is no one to give her the meds or insulin. I panicked and reached there around 9 am and saw that she almost passed out on the bed and she pooped in her diapers because the washroom was locked from inside. Her sugar was very high at 375 and she was showing the withdrawal symptoms because they did not give her the pain medication - pregabalin and no insulin was given to her.I simply brought her back home and reported the same to 911. My mother could have died or gotten into worse condition. She was not safe in that facility. I simply want this incident reported so that this facility should be closed down ASAP and my mother should not be charged via her insurance for putting her life at High Risk. This trauma can never be healed from our lives because of this facility's negligence. They almost killed my mother.
CLOSE DOWN THIS FACILITY ASAP. NEVER SEND YOUR LOVED ONES TO THIS FACILITY.

While the staff are friendly I'm not sure anyone communicates. Between all the staff working with my mom no one recognized a problem and it almost killed her. I would never let any family member go here again.

Many friendly, caring, and kind staff personnel. Much improvement over the year I have stayed here.

There is something to be said when you walk into a community and see smiles and hear laughter. This is what I experience every time I come to Madison Health and Rehabilitation Center.

I stayed here in September 2023 and did not have a great experience. I stayed again from the beginning of September to today, and it has drastically improved. The staff were pleasant and very helpful. The food was nice and much better than last time, even better than where I live. I'm actually going to really miss this place and the people here. I even asked if I could stay longer. Some of my clothes were lost in laundry but the facility made so much effort to find and replace it, the stuff they couldn't replace, they purchased for me and sent it to my home. Leaving here is going to hurt my heart, I feel like I'm leaving my best friends. I came to really care about the staff and love them all. The social worker, Marissa, is especially wonderful. She remembered me from my stay last year. She's bright and cheerful, very friendly, and helped with all my questions and made sure my items were looked for and replaced. I'm going to miss everyone dearly.

About this data
How to read this page
Star ratings vs. Care Quality Score: The Google star rating reflects visitor and family opinions collected from public reviews. The Care Quality Score is an independent metric derived entirely from federal inspection data — the two measure different things, and comparing them side-by-side gives a fuller picture.
Where this data comes from
All clinical data on this page originates from the Centers for Medicare & Medicaid Services (CMS), the federal agency that certifies and inspects every Medicare- and Medicaid-participating nursing home in the United States. Reviews are imported verbatim from public Google business listings.
