I was treated terrifically here! I had no problems, and if I needed something, the staff helped me right away.

The Plaza Healthcare facility was amazing! From the minute, I walked in the door and was greeted, offered a bottle of water and they checked me in and told me how they needed to do that every time I came. Then I went to see my husband, and he was pleased with the care he had been getting. I loved their little diner. I loved how clean they kept this facility and all those that came into take care of Danny and the people that took care of all the planning and the healthcare and being a liaison between the doctors and the dialysis was just phenomenal. We as of yet to know what our insurance will pay and what our portion will be! But they made us feel like family

It was lonely for me because I was mostly in isolation. I did enjoy physical therapy and I liked most of the staff

1/20 2:09am This is now truck NUMBER 3 within the last 1.5 hours. This is ridiculous. The drivers take 6-8 times to dock with constant beeping. I’ve never lived in an area so loud and industrial! 1/14 7:41 am It’s now 7:40 AM and there’s yet another delivery taking place with loud trucks and constant beeping. At this point, it feels like there are more deliveries than actual work or patient care happening, because the noise is nonstop throughout the week. I hear truck beeping far more often than occasional activity, and it’s becoming excessive and disruptive for nearby residents. This seems to be a recurring issue that needs better coordination and consideration for the surrounding community. 1/12 1am Extremely disappointed. It’s currently almost 1am and Plaza Healthcare has BEEN and continues to be a major disturbance in South Scottsdale. There is loud, constant noise at all hours—beeping trucks after midnight and trash bins/wagons being pushed through the parking lot late at night and early morning. The noise is excessive and disruptive, especially for nearby residents trying to sleep. This level of disturbance is unacceptable for a facility in a residential area. I hope management takes responsibility and implements noise-control measures immediately. 1/14/2026. It’s now 4:00 AM 2 days later and once again there’s a delivery taking place with constant truck beeping and excessive noise. The delivery trucks struggle to dock, which drags the noise on even longer. This is the second delivery in just two days, both happening in the very early morning hours. For residents nearby, this level of disruption at 4 AM is unacceptable. There needs to be better scheduling or noise control, because this is repeatedly disturbing sleep and peace in the surrounding area.

Our beautiful 22-year-old daughter was hit by a car while riding her scooter to work. After spending a month in the ICU, she came to Plaza Healthcare for her rehab. This place has more than exceeded our expectations, and everyone here has been an amazing support system for our daughter on her recovery journey. Her doctor, nurses, CNAs, and RTs were incredible and always made her feel truly cared for. They consistently offered words of encouragement and helped her through some of the frustrating moments she faced with her TBI and the healing of her body. Her Speech, Occupational, and Physical therapists were absolutely AMAZING!!! They helped her grow stronger both physically and mentally, and they played a huge role in helping her regain her confidence and independence. The kitchen and housekeeping staff were also wonderful—keeping her well fed with delicious food and her room clean. It was always a pleasure seeing them. Honestly, the entire staff went above and beyond to make her feel special. Anytime she saw someone, they greeted her with a big smile and told her how great she was doing. Not only did they take such wonderful care of our sweet girl, but they also took care of my husband and me. We truly felt like part of the Plaza family and will miss them deeply when our girl comes home. I sincerely hope no one ever finds themselves in need of a care facility, but if you do, I highly recommend Plaza Healthcare. You and your loved one will not be disappointed. We had looked at one other facility, and the staff there didn’t seem happy or like they enjoyed their work. The first thing we noticed about Plaza was how happy and friendly everyone was. When the staff enjoy being there, you know your loved one is in good hands.

About this data
How to read this page
Star ratings vs. Care Quality Score: The Google star rating reflects visitor and family opinions collected from public reviews. The Care Quality Score is an independent metric derived entirely from federal inspection data — the two measure different things, and comparing them side-by-side gives a fuller picture.
Where this data comes from
All clinical data on this page originates from the Centers for Medicare & Medicaid Services (CMS), the federal agency that certifies and inspects every Medicare- and Medicaid-participating nursing home in the United States. Reviews are imported verbatim from public Google business listings.
