I would not recommend this facility. My family member was admitted, based on her emergency room doctor’s suggestion of a 2 week trial period. After 2 weeks, I was in touch with the social worker to see what was the patient’s recovery progress. She said the doctor did not approve of a discharge and that it was necessary. She would not provide further contact information of a doctor or even a name. After another 2 weeks, I inquired of the patient’s progress, yet there was still no information given from a doctor, only the care team. It was reiterated again that she needed discharge approval from a doctor with whom I could not get in touch. Patient expressed multiple times to the staff to be discharged. Meanwhile, the Patient was also very uncomfortable with the neck brace she was given from the hospital. Patient explained many times to the staff that she needed a better fit. No one had told her that she needed to arrange her own appointment at an orthopedic for a neck brace. I finally discovered that we needed to provide an appointment date and arrange for an aid to travel with the patient to the orthopedic’s office. I asked the orthopedic for his recommendation for the rehabilitation discharge, figuring that would help to discharge my family member. Finally after a month and a half, we were able to get patient out of the facility. We only got the patient rights document the day before discharge.

About this data
How to read this page
Star ratings vs. Care Quality Score: The Google star rating reflects visitor and family opinions collected from public reviews. The Care Quality Score is an independent metric derived entirely from federal inspection data — the two measure different things, and comparing them side-by-side gives a fuller picture.
Where this data comes from
All clinical data on this page originates from the Centers for Medicare & Medicaid Services (CMS), the federal agency that certifies and inspects every Medicare- and Medicaid-participating nursing home in the United States. Reviews are imported verbatim from public Google business listings.
